Life certificate

ZUS, the Social Insurance Institution in Poland, sends out a yearly life certificate in autumn to every beneficiary of the Polish pension to prove that he or she is still alive.
If the recipient lost the life certificate, which is called the EMRG, or believes he or she never got one, the form is available below to download.

After the EMRG is completed it needs to be sent to:

Please note: In some cases, a beneficiary’s pension has been terminated because ZUS never received the EMRG back from him or her. If this is the case, it is helpful to also send ZUS a copy of the EMRG via email in addition to mailing it via international post. Currently, ZUS is willing to restart the payment of the Polish pension upon receiving an emailed copy of the EMRG, thus allowing the beneficiary to resume payment by the upcoming payment date. You can email us a copy of the EMRG at contact@polishpensionhelp.org and we will forward it to ZUS. Again, the beneficiary will still need to mail the original form to ZUS as well.
