Every recipient of the Polish pension receives a certificate of life (COL) form from ZUS in autumn of each year to check if he or she is still alive. The form looks like this.
Sometimes the recipient will not receive it due to a variety of reasons (e.g., change of address, it got lost in the mail, etc.), or he or she had received it and mailed it back, but ZUS never got it.
