Several beneficiaries of the Polish pension who mailed in their life certificates to ZUS have received an acknowledgment letter stating that their certificates have been received and confirming their continued right to receive the pension.

As of this writing, ZUS has still not sent out life certificate forms for 2025 and thus encourages beneficiaries to submit their own. You may find sample forms with instructions on how to complete them and mail them back to ZUS here.
